  What means if GMP_NAIL_BITS>0?

Nails are an experimental thing which is not well maintained.

Each limb has not 64 bits but 63 or 62 bits with reserved bits?
GMP_NAIL_BITS>0 is faster or slower than case GMP_NAIL_BITS==0?

It is faster on a few (now perhaps all obsolete) machines with nails.
