It doesn't *have* to be 100% consistent. The interesting thing is that
the next higher function, mpn_div_qr_1, should return the high quotient
limb separately. For div_qr_1n_pi1, it seemed natural to pass a high
limb, uh < d, as a separate argument.

I'll see if I'll get to change qr_1n_pi1 too.
